Reminders:
- None of the medical plans have any employee cost, but if you want to see rates, opt-out amounts, etc. here is the 2024 Rate Sheet
- Deciding between the Low-Deductible Plan or High-Deductible/HSA Plan is typically confusing, so I have also included a financial comparison of the options.
- There is no enrollment form required for Dental or Vision Coverage.
- If any forms ask you for the “effective” date, it is the first day of the month after you get hired (e.g. if you are hired on 7/15/24, it would be 8/1/2024).
- Please try and complete these forms within 5 days after you are hired.
- If you want copies of any of the completed forms, just let us know and we will be happy to provide them.
- Dental Insurance is automatically provided using the information you provide on the medical enrollment form.
