New Hire Enrollment

None of the medical plans have any employee cost, but if you want to see rates, opt-out amounts, etc. Refer to the Rate Spreadsheet above.
Deciding between the Low-Deductible Plan or High-Deductible/HSA Plan is typically confusing, so I have also included a financial comparison of the options.
There is no enrollment form required for Dental or Vision Coverage.
If any forms ask you for the “effective” date, it is the first day of the month after you get hired (e.g. if you are hired on 7/15/24, it would be 8/1/2024).
Please try and complete these forms within 5 days after you are hired.
If you want copies of any of the completed forms, just let us know and we will be happy to provide them.
Dental Insurance is automatically provided using the information you provide on the medical enrollment form.

Deferred Compensation: Per our L106 Union contract you are eligible for the Washington State Deferred Compensation program with a 6.00% matching contribution from the City of Bellingham. There are 3 options for Deferred Compensation. For 2025, the maximum annual contribution increases to $23,500 and you have the option for pre or post-tax contributions.
City of Bellingham Flexible Spending (FSA) Form: If you want to set money aside in a flexible spending account, you will need to complete this form. If you select the HSA eligible medical plan, make sure that you choose the “LIMITED” FSA. If you have questions, view the Staff Central page.
