  1. Notify [email protected] - This will ensure that changes are processed for records across COB and provide you an opportunity to update any beneficiary forms.
  2. Update your LEOFF Trust Enrollment and/or Opt-Outs to reflect the change.
  3. You will need to upload any required documentation to the WFT File Portal and it will be get filed with COB and Premera.
