Step 1: Log in to your Premera Account
If you don't have a Premera Account,
you can do that here
Step 2: From the welcome page, select Submit a Claim (see photo)
Step 3: Select Medical or Vision
Step 4: Complete the required fields
(Date plus 5 clicks mostly Y/N)
Step 5: Attach a PDF copy of your invoice/superbill
Step 6: Submit
Important: For your claim to be approved, the invoice/superbill needs to have several pieces of information. Most providers will include this, but certain providers like massage therapists and mental health therapists do not always include all the needed information (especially diagnosis codes). To get the best chance of your claim being approved, ask your provider for a superbill and specify you need it to include the following information:
You will need to submit a claim for each provider, but you can submit as many dates as you'd like at once—simply check the box stating you are submitting a claim for multiple dates.
